Micro-focused ultrasounds at several depths

My protocol involves carrying out a session of Ultherapy PRIME®, followed 1 to 3 months later by injections of collagen inducers. Depending on the patient’s age and skin quality, 1 to 3 sessions are recommended, each spaced a month apart. The method allows us to lift the tissues in depth, then regenerate the skin by stimulating the body’s natural repair processes. The new collagen and elastin give the skin a smoother, fresher, rejuvenated appearance.
I start with HIFU in order to set the foundations of the treatment and give a long-lasting lifting effect. For ultimate comfort, I inject lidocaine into a few points in the face and submental area prior to treatment.
First, I treat the SMAS by sending microfocused ultrasounds to a depth of 4.5mm. Men often have a thicker dermis than women and their skin is heavier, with an oily hypodermal layer that you need to get through. This is why the work done with the 4.5mm transducer is vital, to reshape and tighten up the jawline and efficiently lift the submental area, which is often too heavy. I follow the SMAS by using the ultrasound scanner integrated in the device. This helps me to optimise the tightening effect in the angle of the jaw, and also allows me to lift the cheekbones by pulling them up towards the tragus in front of the ear. This is an essential element of the lifting effect achieved.
Smoothing and densifying the skin, erasing wrinkles

The second and third passes are carried out with the 3mm transducer to stimulate the fibroblasts and regenerate the skin’s collagen and elastin. We can vary the depth of the impacts by altering the pressure with which we apply the transducer to the skin. I first pass over the skin with a moderate amount of pressure, then I pass over it again with less pressure in order to achieve a lifting effect on the surface wrinkles and folds.
With Ultherapy PRIME®, visualising the ultrasounds in real time during the treatment allows us to accurately adjust the trajectory over the SMAS and the skin’s layers. This way, I can precisely personalise the treatment for each patient, depending on the thickness of the dermal and hypodermal tissue, and the depth of the wrinkles.
Biostimulation for densified, revived and even skin
The second part of the protocol is scheduled 1 to 3 months after the Ultherapy PRIME® session.
It involves using a cannula to inject a blister of Radiesse® diluted with sterile saline solution to a ratio of 1:2. The higher volume of product enables us to cover a larger surface. I start from the outer cheekbone and inject lines of product in a retro-tracing pattern up to the nasolabial fold then the expression lines. Next, I focus on the jawline, to tighten it up and make it look firmer and more dynamic.
The procedure can be repeated 1 or 2 times, spaced a month apart, depending on the skin quality and appearance.
Patients like this medical lift because it is carried out in two appointments, thus avoiding one long session and giving results that appear gradually over time. The Ultherapy PRIME® treatment is pain-free thanks to the local anaesthetic. The results start to be visible after a month and keep improving for up to 3 to 6 months. No downtime is required, which is an important criteria for male patients. The additional injection of Radiesse® gives more definition to the lift, while improving the skin quality for the long term and making the complexion smoother, more even and more radiant.
In my patients’ follow-up appointments, I have noticed that the results are stable for up to 18 months. The protocol can then be repeated, with the aim of preventing sagging and maintaining the effects over time.
