The IBSA Derma Approach by an International Expert
Dr Eugenio Broccaioli
The lips reflect the identity and emotions; hyaluronic acid fillers are an ally to enhance and care for them. As a visible reflection of both ego and emotion, nowadays we are moving towards a more natural aesthetic for the lips. Hyaluronic acid fillers offer a new language: correcting without transforming, harmonising without exaggerating, and revealing each patient’s uniqueness while respecting their identity.

Lips: At the Crossroads of Aesthetics, Emotion, and Identity
Lips hold a unique place in facial harmony. They are not just an aesthetic feature: they shape speech, play a role in seduction, transmit subtle emotions, and reveal a visible part of our ego. Their symbolic importance explains why they attract so much attention in aesthetic medicine. Over time, patient expectations have evolved. The excesses of the past—artificial volumes—have given way to a quest for authenticity. Patients no longer ask for “augmented lips,” but for lips that stay true to their personality.
The Impact of Society

In an era dominated by selfies, social media, and video calls, our relationship with the face has changed. Young adults often seek early corrections to prevent asymmetry, while people over forty want to restore freshness. But everyone shares the same request: avoiding excess. The fear of frozen, artificial-looking lips is real. The guiding principle is clear: natural results—noticeable, but discreet.
Hyaluronic Acid: A MIXTURE OF Trust and Distrust

Hyaluronic acid (HA) remains the reference molecule thanks to its biocompatibility, reversibility, and safety profile. However, its reputation suffers from overuse and poor results made visible in the media. Not all HA products are the same: molecular purity, manufacturing processes, and rheological properties determine the final outcome. Rheological parameters (viscosity, elasticity, cohesivity, and diffusion coefficient), along with HA’s molecular weight and concentration, predict the aesthetic result both at rest and in motion.
Product choice makes all the difference.

Rheology and Emotion
In lip treatments, rheology is not just a technical concept: it directly influences both aesthetic and emotional perception.
– A filler with low G’ hydrates and softens, creating a glossy effect that patients appreciate.
– A higher G’ provides structure and projection, reshaping the lip contour.
– The tan δ indicates diffusion and reassures us about the longevity of the desired result.
Too much rigidity freezes expression, while too much volume creates a caricature. Here, science and psychology intertwine.
The IBSA Derma Approach and the Aliaxin® Range
My international experience has allowed me to compare many products. IBSA Derma stands out for its pharmaceutical stringency and vision centred on authenticity. In the Aliaxin® range, two references dominate for lip indications:
– Aliaxin® FL (Fine Lines & Lips): molecular weight of 500 kDa and 1000 kDa, concentration of 25 mg/ml, low G’ (45 Pa), tan δ of 0.39. Ideal for subtle hydration, softness, and grace.
– Aliaxin® LV (Lips Volume): molecular weight of 1000 kDa & 2000 kDa, concentration of 25 mg/ml, higher G’ (107 Pa), tan of δ 0.21. Designed for support and projection, with harmonious tissue integration and a more structured result.
In my practice, I prefer not to combine Aliaxin® FL and Aliaxin® LV, as I believe the outcome would be too noticeable. I adapt the technique—needle and/or cannula—according to the indication, ensuring precision and safety.
A Growing Demand Among Men
A clear trend is emerging: more and more men are seeking lip treatments. Their motivation is to correct signs of aging while keeping results discreet. Again, the goal is subtlety—proof that aesthetics now transcends gender and reflects a universal pursuit of harmony.
Beyond Technique: Building Trust
Injections go beyond the medical gesture. They require listening, education, and transparency: explaining HA reversibility, emphasising pharmaceutical quality, and reassuring patients about their concerns. This dialogue transforms the treatment into a positive experience and strengthens the practitioner–patient relationship.
Authentic Beauty: A Philosophy
With its Ego Lips project, IBSA promotes a strong vision: enhancing without transforming. Every mouth reflects a deeply personal identity and deserves an individualised approach. For me, aesthetic medicine should not impose a universal standard but rather reveal each person’s uniqueness. The future of fillers rests on three pillars: pharmaceutical purity, technical precision, and respect for identity. With Aliaxin®, science and art come together to restore the lips’ essential role: expressing emotion, authenticity, and individuality.
