The Coolpeel© Protocol: An interesting alternative to light peels?

A dedicated, specific protocol using the microablative fractional CO2 laser allows for non-invasive photorejuvenation treatment with minimal downtime: the Coolpeel© protocol.

Technology

The machine is primarily used in CO2 laser mode (wavelength 10600nm). The unique aspect of this treatment lies in the combination of two technological principles:

• High Pulse (HP) mode, which generates extremely short pulses and high peak power.

• Scanning mode in “spray” mode, which randomly distributes the impacts within the targeted area, preventing overlapping pulses and creating epidermal pits called DOTS.

The ablation creates pits 70 to 120 microns wide and 32 microns deep, with a coagulation depth of 80 microns. All skin phototypes can be treated. Two protocols are available to the practitioner, provided they have the laser in its dual-wavelength version, CO2+1540 (Duoglide©). In the CO2-only version, the Coolpeel© will only affect the epidermal area. The stimulation of deeper layers, secondary to the heating of superficial areas, generates an interesting, though moderate, post-treatment reaction. In its CO2+1540 version, the superficial dermabrasion action of CO2 is complemented by a deeper thermal action with fibroblast and collagen stimulation. The glow effect will be more pronounced, especially on younger skin. The lightweight and maneuverable handpiece, combined with a multi-articulated arm, allows for the treatment of a full face in 10 to 15 minutes, and a neck or décolleté in 10 minutes.

Indications

The main indications are:

• Skin photorejuvenation.

• Complexion improvement.

• Radiance boost with a glow effect.

It is important to note that this protocol obviously targets the face, but can also be performed on the neck and décolleté.

Several options are available, depending on the individual’s expectations:

• Coolpeel© Light: Only the CO2 laser is set to a low intensity configuration, 0.8-1 W, with a standard spacing of 900 microns between each well.

• Coolpeel© Classic: The CO2 laser power is increased to 2 W, used as CO2 alone for processors on a standard platform, but can be combined with the 1540 Laser for its revitalising effect.

• Coolpeel© High: The power increases to 3 or even 4 W, and the spacing is reduced to 800 or even 700 microns. And the benefit of the 1540 Laser is clear in creating subcutaneous matrix stimulation (Photo 4).

Coolpeel© versus peels

The Coolpeel© in its light version (0.7 and 1 W) can be considered a rejuvenating peel: immediate, transient effect, like a temporary radiance boost. A classic Coolpeel©, in its CO2-only version, at 2 W, can be compared to a light peel, such as a 12% or 15% TCA peel. The effect on skin quality and texture, on certain light spots, and on fine lines is identical. If the parameters are more intense, with the combination of the 1540 Laser and repeated passes over certain areas (spots, fine lines), Coolpeel© then offers results equivalent to a multi-layered 15% TCA peel.

Coolpeel© creates a complementary approach to skin care and integrates perfectly into a versatile aesthetic medicine practice. However, it will be less recommended for individuals over 65, those with a “Glogau 3” (a specific skin condition), due to skin damage, the depth of wrinkles, or if the individual has a strong desire for skin resurfacing.
